A chin that looks dimpled or pebbled at rest is usually muscular, and responds well to a small, well-placed dose.
Some chins look uneven, pitted or pebbled at rest, an appearance sometimes described as orange peel chin. It is usually caused by the mentalis, the muscle in the chin, contracting in an irregular pattern and pulling the overlying skin into small depressions.
Treatment places small amounts of botulinum toxin into the mentalis to relax that pattern. The chin surface smooths, and the skin sits more evenly.

Small doses are placed into the mentalis, usually at two or three points, at a depth chosen to relax the muscle without affecting how the lower lip moves.
The constraint is lower lip function. The mentalis contributes to lip position and to how the lower lip everts when you speak, so dose and placement need to relax the dimpling without altering speech or lip movement. Placing too superficially or too close to the lip border is what produces those problems.
Dr Sateei also assesses whether volume loss at the chin is contributing, because a chin that has thinned can look more textured, and in some patients a small amount of filler addresses the appearance better than muscle treatment does. Where both are involved, the muscle is usually treated first and the result reviewed.
